Sex Hormone Binding Globulin (SHBG) is the transport protein for circulating testosterone and estradiol (E2). Although total hormones are measured, only the free hormones are biologically active. Measurement of the SHBG allows the free hormone to be calculated (known as the free androgen index).

Free androgen index (FAI) = 100*(total testosterone/SHBG) 

In conjunction with testosterone, measurement of SHBG and calculation of FAI can be used in the investigation of polycystic ovarian syndrome (PCOS) and erectile dysfunction.
