Interpretation during bisphosphonate monitoring:
At baseline:
– If P1NP is >80 ug/L, then unless recent fracture, seek cause of high bone turnover.
Response at 6 months:
– Good: <35 ug/L and/or fall >10 ug/L
– Suboptimal: >35 ug/L and fall <10 ug/L

P1NP (Type 1 procollagen amino terminal peptides) is used as a marker of bone turnover.
These N-terminal extension pro-peptides are released into the blood during type 1 collagen formation. As type 1 collagen is an important component of bone matrix, P1NP levels act as a marker of bone formation. Elevated levels are found in patients with increased bone turnover.
