Cellular uptake of iron is controlled by expression of the transferrin receptor on cell membranes. In plasma, soluble transferrin receptor (STFR) forms a complex with transferrin. Levels of STFR are directly proportional to membrane-bound transferrin receptors.
SFTR is measured alongside ferritin in the investigation of iron deficiency and a STFR-ferritin ratio is calculated:
- <1 iron deficiency unlikely
- >2 iron deficiency likely
- Ratio 1-2 AND raised SFTR – check haemoglobin possibly a short course of iron
