Troponins are proteins that form a complex which control the interaction of actin and myosin to regulate contraction in striated muscle. Skeletal and cardiac isoforms are distinct from each other. Therefore, the cardiac isoform of Troponin-T is a specific marker of cardiac damage which is used in investigation of acute myocardial infarction (MI), particularly in non-ST elevation MI (NSTEMI). A rise or fall in troponin-T levels may indicate cardiac damage, although it is not specific for MI. Troponin levels may remain elevated for 2 weeks after an MI.
The laboratory uses the Roche high sensitivity troponin-T method (hs-cTnT) which can detect changes 1 hour post MI. Measurement of troponin-T should be based upon local trust guidelines and protocols.
